The person named above is a faculty or staff member of the University of Waterloo. By accepting a copy of this product, this person agrees to the following conditions governing its use:

  1. This product will only be used on University-owned machines and only for research, teaching or administrative purposes. It will not be used for any commercial reasons.
  2. Recipient will not make any copies of the software. Backup copies will be retained by IST.
  3. Any copies of the software Recipient has in his/her possession will be returned to the IST Computing Help and Information Place (CHIP), MC1052 if he/she leaves the University.
  4. In order to receive your software you must take this signed form to Computing Help and Information Place, IST, MC1052.
