The person named above is a faculty or staff member of the University of Waterloo. By accepting a copy of any of these Autodesk products, this person agrees to the following conditions governing its use:

  1. This product will only be used on University-owned machines and only for research, teaching or administrative purposes. It will not be used for any commercial reasons.
  2. This person will not make any copies. Backup copies will be retained by IST.
  3. Any copies this person has in his/her possession will be returned to the IST Computing Help and Information Place (CHIP), MC1052.
  4. If this person is an administrator of the site licensed copies of Autodesk products and leaves the University, he/she will notify the CHIP at x3456, of their departure and replacement.
  5. If a product is being purchased for a network, the number of copies required is determined by either the number of machines or the number of controlled accesses.
  6. Any machine readable copies of documentation may be printed for your use only and may not be copied and given to others.

In order to receive your software you must take this signed form to Computing Help and Information Place, IST, MC1052.
