The person named above is a faculty or staff member of the University of Waterloo. By accepting a copy of this Cognos product, this person agrees to the following conditions governing its use:
  1. This product will only be used on University-owned machines.
  2. No copies of the software will be made.
  3. Any copies of the software this person has in his/her possession will be returned to the IST Computing Help and Information Place (CHIP), MC1052, upon leaving the University.
  4. If this person is an administrator of the site licensed copies of the Cognos software and leaves the University, he/she will notify the CHIP of their departure and replacement.

In order to receive your software you must take this signed form to Computing Help and Information Place, IST, MC1052.
