The person named above is a faculty or staff member of the University of Waterloo. By accepting a copy of any of these Microsoft products, this person agrees to the following conditions governing its use:

  1. This product will only be used on University-owned machines.
  2. This person will not make any copies.
  3. Any copies this person has in his/her possession will be returned to the Computer Help and Information Place (CHIP), MC1052, University of Waterloo, upon leaving the University.
  4. If this person is an administrator of the site licensed copies of Microsoft products and leaves the University, he/she will notify the CHIP of their departure and replacement.
  5. If a product is being purchased for a network, the number of copies required is determined by either the number of machines or the number of controlled accesses.

In order to receive your software you must take this signed form to Computing Help and Information Place, IST, MC1052.
